1- it's cheating and I disagree with cheating.
2- It could taint the group we were in, the club, and possibly the country.
3- It would potentially stealing a place from another person.

Yes. I would tell him to do it first though and that if he didn't do it that I would. To save the sport and more importantly, him.
And actually, when thinking about this more, what I'd probably do is tell him that he is not to run in the trials. I probably wouldn't turn him in as long as he stopped doping and didn't run in the trials all doped up.

If I knew a friend that were in possession of marijuana I would not go to the police. I don't use it myself.
A friend taking PED's? Same thing. It's not for me but I really don't see myself turning him in.
I have no knowledge of anyone I know taking PED's but I am less inclined to do something about that than a more serious matter.
If I knew a friend raped his girlfriend or assualted someone then I can see getting involved.
I may allow a range of moral and ethical standards to the point where someone is not really getting hurt before taking action.
Beating someone in a foot race does not fall out of that range.

Regarding # 3 on your list: If he made the team, then, yes, I would turn him into the authorities which would open up a spot for the next person.
Other than that, no, I would not.
They would no doubt ask for proof before they react. How would you document it? This is assuming he/she passed the drug test because I think they already test the top finishers in the Oly trials.
Its the trials so he would not be taking a spot from anyone. Unlimited slots, just have to run the time. But no, I would not turn him in
Yes. I live a life of absolutes when it comes to morality. There are no shades of gray. I do not associate with cheaters or liars.
As a fellow athlete, he has cheated against me. If he is a cheat, then he is no longer a friend. As a result, any bizarre, pseudo crime family "no snitches" loyalty would not be necessary.
