I don't. I just pay attention to my surroundings, keep an eye out for headlights, etc.

Yes...usually a vest. All shoes have some sort of reflective piping...I would highly recommend wearing something...
I do on the morning runs that are usually in my
neighborhood. Lots of people are driving at 5AM
there and a lot probably aren't expecting to see
a runner that early.
I also think that a lot of older people's night
vision is pretty poor and that population group
seem to be the most surprised to see runners
in the wee hours of the morning.
An ounce of prevention and all that rot.
I'd rather not advertise my presence to the rapists and muggers eagerly licking their chops at the opportunity to take advantage of me.
I just try not to run in the middle of the street when i see two large moving orbs of light headed in my direction.
I recently took a left turn at night onto a side street and nearly clocked a guy who was crossing the street. He had no reflective clothing, lighting, etc.. Fortunately my co-pilot saw him and warned me before any damage was done. He was dumb and lucky.
A lady jogger was killed last week in our town at 5:15 in the morning by a drunk driver. She was wearing reflective clothing but it was not enough. The driver already had one conviction for DUI.
I found that also wearing a headlamp significantly increased the time when I was (I don't run at night anymore) seen by on-coming and turning cars. Once you get over the real-runners-don't nonsense, it was great.
