Some background - around two years ago I had a knee pain arise on the lateral side (just on the outside of the patella, not IT band area) during a long run which built up over the course of around half a mile, and resulted in such pain that I had to walk back. The two different physios I then saw put this down to the typical runners knee / patella injury, and treated it with a typical gradual loading and return to running. Through several failed attempts of getting back to running where I broke down, after around 8 months I managed to get back into fulltime running.
For around 6-7 months last year I was managing 40-50mile weeks, until one run last summer where I felt a similar build-up of pain in the same area during the run, although not resulting in having to walk. After returning to the physio I dialed back on running and proceeded with the same gradual loading / to try and return to running, but things have only been getting worse since then. In December I did manage to get up to 10-15miles/week for a short period, but then broke down during a run (a very sudden "stab" of pain).
Right now the pain is such that I feel it a lot of the time even when walking, there is some noticeable mild swelling, and I cannot run more than 50 yards without the stabbing pain arising.
In the last two months I have been to see a specialist who performed an ultrasound and saw no major signs of trauma, and said the patella tendon looked fine. We then did an MRI and again nothing major showed up - tendon, surrounding tissues and meniscus looked fine, however there are some possible signs of degeneration in the cartilage in that area. We are proceeding to try out Hyalorunic acid, but I get the feeling this is more of a hit and hope approach than anything else.
One of the major symptoms I encounter which my physios have not really explained is if I completely unload the leg and stand with the injured leg in a "mid swing" position (heel pointing up, quad driven backwards), then I can induce significant pain in the area. Doing this can often lead to residual discomfort for the following days. There is basically zero pain induced by any physical pressure on the tendon or surrounding area. The source of pain also tends to feel quite deep, but is difficult to place exactly.
Anybody have any further ideas what the actual injury here could be? Do you think the doctors are likely correct in their prognosis of degenerative cartilage being the cause?
