If your wife is inevitably wrinkly and chubby, you say her hair or outfit looks nice. You can say that her plan for the day is excellent, or the meal she cooked is superb. You can say that her idea is great even if you don't care one way or another. If she mentions something obvious, say wow, glad you thought of that. The smarter she is the more subtle you need to be. Most of us won't have to worry about that.

If this were FaceBook, I'd give your post the "love" emoji. My wife and I have been married for 30+ years. I send her a text or two throughout the day to tell her that I love her and tell her that I hope that she's having a wonderful day. I call her as soon as I get out the door from work to let her know that I'm on my way home and that I miss her / love her. I compliment her on looks occasionally, but more often thank her for what she's done, letting her know that I'm appreciative. On the weekends she usually fixes me breakfast - pancakes or omelets. She ask "how's your breakfast?" and I always reply "best pancakes/omelet ever!". When I come home from work, I greet her with a hug, kiss and a "love you".
I am quite infatuated with a younger coworker and take every opportunity to compliment her. I am a bit afraid that she sees it as cringeworthy given my age, but I would leave my wife in a second if she would have me. I hope she is not mocking me behind my back with her friends.

I compliment my wife on her appearance when she's actually looking nice, which happens about once a month. I complement my girlfriend on her appearance every day.
Compliments are given when compliments are deserved.

Note the OP said compliment her ON HER APPEARANCE. This seems to me a bit tricky. On the one hand, it strokes the ego of females to be told they are pretty/beautiful so they like to hear it. On the other hand, inevitably, there will come a day when they are no longer pretty/beautiful. And so if they get the message that the only reason you are with them is because of their looks, it could make them insecure in the relationship in that they will worry the day will come when you will trade them in for someone younger and prettier.
