I do traditional Thai massages, which are clothed. But when I do the other kind, I do keep my undies on. I guess if you have lower back issues what you're doing is ok. Otherwise ... kind of weird.
Also, maybe PT for your back issues would work better.

Why would you wear underwear? There would be oil all over it. And it would rub against your skin as the massage is occurring in that area. Makes no sense

Exactly. Honestly, your massage therapist in their head will think it is weird keeping clothes on.
Also, you might think it is creepy to take your clothes off but you are making it awkward for them by keeping them on suggesting that there would be something inappropriate about removing them.

As a massage therapist I can tell you that wherever I work the long time clients all go with out underwear as honestly is the norm. Athletes as well. It is very difficult to work the back with bra straps in the way or getting into the glutes and IT band with elastic bands in the way. And the oil gets everywhere. There is really no sense to come in with underwear on. Your bottom is always covered with a blanket. That is the rule for therapists. It benefits both sides to just take them off

I wear running shorts. I'm not particularly modest, but my massage therapist is female, and I don't want any confusion. When the shorts are in the way, she adjusts them.

I have waited a long for this topic to pop up. After I won Mardi Gras Marathon I went to this place for a massage. They said we don't do massage here.
Ahhh that was you. Well, you know what you did wrong and we certainly don't want you to come back. Try and find another massage parlor but they will most likely also ban you.

I mean, do whatever is comfortable for you, but do you really think that it de-sexualizes the situation to make your massage therapist play with your waistband and possibly have to pull down your shorts herself during the massage?
As a physician, I'll always ask a patient to remove or manipulate their clothing themselves if they are able rather than doing so myself when "sensitive areas" need to be accessed. It would feel way more invasive to yoink someone's underwear down myself than to just have them undress before I begin the exam. To me, the act of manipulating someone's clothing only increases the awkwardness and the odds of sexual intent being perceived by one party or the other.
