You completely missed the point. What we need is nutritious animal-based foods with 100% bioavailability, sustainably pastured, to replace the crop-based, toxic flours, sugar and oils. This has to change from the grass roots, with people boycotting the junk that is killing them, by educating themselves on proper nutrition rather than following government and corporate dogma and advertising.
None of these modern engineered foods existed 120 years ago, and there was essentially no heart disease, cancer or obesity, regardless of other factors like activity level.

White flour has been used for about 150 years in a form somewhat similar to the modern form. How many people had access I have no idea.

The seed oils did not. Crisco was the first industrial seed oil product introduced in the 1920s. In the 1930s heart disease rates began to climb. Later on seed oils like canola were created as well as hyper processed corn, peanut, sunflower etc., which resemble a natural oil like olive oil in name only. They are essentially industrial waste products weaponized and utilized to jack up profitability while infiltrating almost every food product in modern society. Read any typical food with over three ingredients and you will find sugar and seed oils in there where they were in in in our great-grandparent's time.
Flour and sugar were around of course, but were not essentially free to the food industry due to subsidies introduced in the 1970s. Also in the late '70s the federal government essentially condemned the most nutritious human foods: meat, eggs and dairy which contain saturated fat and cholesterol, which are essential to human health. This was based on skewed research and the "beleif" of those calling the shots that these foods were "bad for human health". This shifted food production to a "low saturated fat/high carb" ratio of equal parts refined carbs and seed oils, which created and promoted hyperpalatable foods with essentially no satiety (i.e. are insatiable to humans). Read on the linked website for more info on how these modern foods undermine your body a the cellular and systemic level.

There is good news.
Heart disease remains an epidemic, but deaths from heart disease have been cut by greater than half over the past 50 years. Perhaps by the shift to less saturated fat but more likely due to medical advances, most notably the advent of statins among other major developments.

Whether it increased is hard to discern. Over the past 150 years people are living longer, less likely to die of other things, and more likely to be diagnosed correctly.
But the good news is cancer survival rates have gotten much better and cancer death rates in the US are falling.

In the US we've cut sugar over the past few decades, but obesity continues to rise. The overall macronutrient composition has remained about the same for 60 years, we just eat more overall. The rest of the world is getting more money, eating more meat and fat, and getting fatter. The only skinny countries now are where they're too poor to afford anything but rice or a few prosperous Asian countries who haven't changed diet all that much.
