No one can guarantee that running dozens of throwers through that no one will slip or lose grip or whatever. That's the the job of the tall netting around the discus circle. It should be set up to block all the angles that would be dangerous, but obviously it wasn't sufficient to cover all those angles. I'd blame the setup of those nets.
