Middle school isn't the right time because they should start earlier. That way, they can move very slowly and gradually before they have to make any medical decision.
Jamie Bruesehoff's book I posted above explains how she did this with her daughter. They started removing some boundaries very slowly and gradually. There are boys who like dressing up as girls or playing with Barbie dolls. If the kid is not trans, they will stop it somewhere along the line. Jamie's daughter never stopped. She became better and better adjusted to life as they moved along, and her mental health improved. (You can see how this happened in real time in her FB page. It's a fascinating reading.)
So by the time she took the first dose of puberty blocker she was already living as a girl for two and a half years, and there was no doubt on how stable her gender identity was. And it's not just her. A kid who expresses their gender incongruence in early childhood (3-4 years old) and never changes to the onset of puberty is not going to change after that. That's why it is important to see medical professionals early on, and closely work with them for years before they start taking any medicine.
