Never paid much attention to that aspect of their opinions. These "antivaxxers" you talk about. Who are they ? Not me. I think its great that people in high risk groups have the option of choosing an experimental vaccine to protect them from a potentially fatal virus. That's not going to happen in the Republican controlled states. You have point with the Democratic controlled states especially the heavy Leftist states of NY, CA, WA, etc. You already have an authoritarian passport system in NYC & San Fran that doesn't allow a testing opt out, and a passport requirement in LA that does (surprisingly) allow a testing opt out.
You won't find any passport system in any Republican state except in New Orleans, where the Democratic mayor authorized the passport which does include a testing opt out. However, Louisiana state legislatures are currently working on a bill to ban any passports in the state (which should pass with the Republican controlled House/Senate).
Keep in mind that Republican leadership highly recommends the vaccine but doesn't believe in any government mandates & honors informed consent & medical freedom. Democratic leadership simply wants to remove a person's choice in any medical procedure/intervention such as vaccination and supports medical authoritarianism. This will just divide the country even further - it will get far worse before it's gets better.
