At a MINIMUM, for a child under the age of 18, it should require parental approval (and I would call that bad parenting), and ideally, this should not be allowed to be done by doctors until the person is 18 or older.
One thing to have a person under age 18 identify as a different gender, but surgery is permanent. No permanent decisions like that should be allowed to be made by anyone under age 18.

I don't know. Can't drink in the US until you are 21 or vote or join the military until you are 18. Having gender reassignment surgery before age 18 seems to be irresponsible enough that I think there should be a law prohibiting it.
