ER wait times grow as staffing shortages strain area hospitals
Officials say the state's vaccine mandate is compounding the shortages at a critical time

Sick and injured patients across the Capital Region and Hudson Valley are waiting hours to be seen in emergency rooms as a growing shortage of health care workers around the state has forced some hospitals to hold incoming ambulances in bays for hours or turn them away altogether.
Hospital leaders say the situation has not yet affected patient care but worry it may eventually if the staffing crisis worsens or the current surge of COVID-19 patients grows bigger. "The truth of the matter is because of the number of patients in the hospital our ability to take (transfer) patients is limited and that, quite frankly, is the exact same thing we are hearing from every other hospital across the state," Albany Medical Center President and CEO Dr. Dennis McKenna told the Times Union last week.
The shortage - which many expect will worsen as a result of the state's vaccine mandate for health care workers - even prompted one area sheriff to publish an advisory this week warning the public of the issue. "So not to cause panic but we need to be cognizant of our healthcare status," Albany County Sheriff Craig Apple wrote in a Facebook post published Wednesday afternoon. "We have a busy EMS Division, and on many occasions our ambulances are being held at the hospital for lengthy periods of time due to not being able to get into the ER." Apple told the Times Union Thursday that his ambulances are being forced to wait one to two hours to get their patients into local emergency rooms, and sometimes even longer. Not only does it create voids in coverage for the region, he said, it hampers his department's ability to provide mutual aid to surrounding areas. "I had a guy the other day tell me that they were sitting outside (a hospital) for four hours," he said, of one of his EMS workers. "That is the most unacceptable thing I've ever heard. It's insane." In his Facebook post, Apple said he's shifting resources to make up for the coverage lapse but worries he can only do that for so long. "Not so bad until a neighboring ambulance is being held at a hospital as well and the shift and void continues," he wrote. "It's a whole trickle down from rehab centers not having staffing so the hospitals can't get patients into them therefore they can't empty ERs." Apple told the Times Union he felt compelled to publish the advisory after a trauma doctor warned him the situation was reaching a crisis point.
In a related impact, Albany Med temporarily closed two of its urgent care locations in Guilderland and Mechanicville as it seeks to "decompress" pressure at the hospital's emergency department. When asked how long the closures would last, spokesman Matt Markham replied, "we're continuously evaluating the situation."
The closures come after the state's vaccine mandate for health care workers became enforceable Monday and Capital Region hospitals, including Albany Med, put hundreds of employees on leave without pay for their refusal to comply. At a Tuesday news conference to discuss the state's vaccine mandate and its impact on staffing, McKenna described a vicious cycle of worker shortages feeding off each other. Because nursing homes have shortages as well, he said, hospitals that need to discharge patients to skilled nursing facilities are being forced to hold patients in beds longer than necessary until they can find an available nursing home bed. While the vaccine mandate did not create the shortage, he said, it is expected to exacerbate it. McKenna said roughly 204 of over 11,000 employees in the hospital system failed to get their first vaccine dose or an exemption by the deadline, leading to their suspensions. Of the 204, 10 were physicians, 48 were nurses, 29 were patient care associates and six were students.
