Not necessarily falling, but Bekele in WXC 2008 moved off course in the first mile (looked more to be a shoe silpped off and he had to re-lace it).

Not sure. Steve Jones answered natures call in the London Marathon and still caught Charlie Spedding
I think Grete was having issues in the NYC Marathon one year, and she may have ducked into an alley to relieve herself.
Paula Radcliffe just squatted in the street and dropped one.
Back to the falling, and since the marathon women have been mentioned, I will mention the 1992 women's Olympic trials marathon. At a water stop around the 16 or 18 mile mark, Janice Klecker got tipped up and went down. Cathy O'Brien had to slow and hop over her. O'Brien took another step and a half, turned around and helped Klecker to her feet. Klecker went on to win, with O'Brien second.
Note that my memory is fuzzy on that and the roles may be reversed.
I've been trying, without success, to find the classic T&FN photo--but anyway, in the 1976 Olympic Trials Miguel "Mike" Roche of Rutgers fell on the last barrier in the steeplechase and was maybe 15m behind Don Timm--caught Timm before the line and grabbed the third spot on the team. The photo had Timm running for dear life, with Roche getting to his feet and looking at Timm with laser intensity.
YouTube has a video from that meet, but only of Mike's first-round race, not the final.
The real question is: Has anyone ever been lapped in a race, fell and still came back to win?
I had a friend who fell down in a marathon on a track. He ended up getting lapped and came back to win.
