you guys are sick of hearing this, but Kinvara 2. It didn't directly cause injury, but made my feet so weak i got easily injured during regular activity

Nike Jasari (training shoe). The heel was so high in the back that it rubbed my achilles raw!
A close second would be the short lived Pegasus Racer. At the time, I wore the Skylon and it was lighter and built more like a racer. The purchase was made online and boy was I disappointed when they came in!

The Puma Ryjins. Jesus they are an awful shoe. My feet would collapse towards the middle, as if the sole wasn't positioned under the upper correctly. They also caused me ridiculous ankle pain. I have never seen so many people injured at once as when my team decided to give these shoes a shot.
