Have gotten dozens and dozens of massages over the years. Always wore the loose Seb Coe-like running shorts. Plenty of room to maneuver around. Glutes, IT band, no problem.
Why would you wear underwear? There would be oil all over it. And it would rub against your skin as the massage is occurring in that area. Makes no sense
So if oil gets on it, you wash it later. Or is laundry also something you omit?

The majority of people completely disrobe. There is absolutely no point to wear anything. You are completely draped at all times except for the part they are working on. I’m shocked at these comments.
Pretty much never. I had one sports massage therapist say that if I come back she'd recommend I wear shorts so she could do a little more stretching. But every other time I've gone nude and it's been the norm.
I wear running shorts. I'm not particularly modest, but my massage therapist is female, and I don't want any confusion. When the shorts are in the way, she adjusts them.
I mean, do whatever is comfortable for you, but do you really think that it de-sexualizes the situation to make your massage therapist play with your waistband and possibly have to pull down your shorts herself during the massage?
As a physician, I'll always ask a patient to remove or manipulate their clothing themselves if they are able rather than doing so myself when "sensitive areas" need to be accessed. It would feel way more invasive to yoink someone's underwear down myself than to just have them undress before I begin the exam. To me, the act of manipulating someone's clothing only increases the awkwardness and the odds of sexual intent being perceived by one party or the other.
“Ok, Ms. Smith, we need to do your pelvic exam. In order to make this as comfortable as possible, I ask that you leave your shorts on so that I can slip my fingers inside of your waistband and slide them off myself once you’re laying back with your legs spread in the stirrups. Y’know, to avoid any awkwardness.”
I always go completely nude under the covers during my massage therapy appointments for my lower back issues. Recently I asked a few friends and all of them wear underwear… does my massage therapist think I’m a creep? Before talking with them, I always thought everyone stripped down completely.
I've typically worn shorts but it's a modesty thing for me. I will usually wear the most minimal running shorts I have so the masseuse has less to work around and doesn't have to bother with moving the towel/sheets during the massage. And, I don't usually have massages that will be impacted by the presence/absence of shorts or underwear.

I've had a massage done by a masseur, and had it done starkers. If anything, the masseur did a better job than a lot of masseuses, as he was a rather strong fellow. The funny thing here is that nobody is acknowledging the elephant in the room. If you want a dirty massage, you can generally use some common sense in finding a place for that sort of release. If you want a non-dirty massage, you can also use common sense in finding more legit places.
